Non-Member Services

We offer select standalone medical services for non-members, including DOT physicals and pre-operative exams. These services are available at a flat, one-time fee.

Members may receive these services at no additional cost depending on their membership level.

DOT Physical

Department of Transportation Physicals
$ 150 Fee for Non-Members
  • Comprehensive physical exam
  • Detailed review of medical history, medications, and past surgeries
  • Discussion of chronic conditions (diabetes, hypertension, sleep apnea, heart disease)
  • Review of lifestyle factors such as tobacco and alcohol use
  • Vision & hearing testing
  • Blood pressure & pulse check
  • Urinalysis
  • Completion of DOT paperwork (Medical Examiner's Certificate)

Pre-Op Exam

Preoperative physical exam
$ 200 Fee for Non-Members
  • Comprehensive medical history review
  • Physical examination to assess surgical readiness
  • Review of current medications, supplements, and allergies
  • Vital signs assessment (blood pressure, heart rate, oxygen levels)
  • Required lab work and diagnostic testing coordination (as ordered)
  • Medical clearance documentation for your surgeon or facility
  • Identification of potential risks and optimization recommendations
  • Direct communication with your surgical team, if needed
