When you click on the button below, labeled, Join Now, you will be taken to Atlas.md. This is the computer program we will be using to organize our practice, and keep track of your medical history.
Once in Atlas.md, you will be guided through the process of creating an account with Docs by the Bay, PLLC. You will need to enter your demographic data, answer some basic health questions, and give us your credit card information.
You must review the Patient Agreement before you begin this process. This is a very important document that defines our relationship. It is not too long and was intentionally written in plain, easy to read, language.
The Patient Agreement explains our simple pricing plan. When you have completed the above registration process, your credit card will be charged a $100 registration fee. Subsequent charges are defined in the Patient Agreement form.
Once you have completed this form, you will be contacted by our Office Coordinator within a few days to set up your initial appointment!

As a token of our appreciation for your hard work and dedication over the years, Docs by the Bay offers a special discount exclusively for healthcare workers and their spouses. This discount includes waiving our registration fee and offering a 50% discount towards our monthly fee. This is our way of saying thank you for all that you do. We understand the challenges of your profession and are here to support your health and well-being.
Many Healthcare workers are healthy and only need an Annual Physical. This 50% discount also applies to our Limited Membership.
