One comprehensive annual physical exam and history
One set of lab tests: CBC, chemistry panel, cholesterol panel, urinalysis, and EKG (HbA1c and/or TSH if clinically appropriate)
DOT Physicals available ($150 for non-members)
A 3-month membership with our practice (beginning at the date of your physical exam), with all the benefits of a full membership.
You will be considered an active patient of our practice for 14 months, from the date of your physical exam.
Option to convert to a full membership anytime (no reactivation fee if within 14 months)
Call the office for simple questions; for more involved needs, you may upgrade to full membership
Convert within first 3 months? Membership billing won’t start until 3 months after your physical
$350 annual fee
